Equipment Lending Form

Book CIS equipment and sign the appropriate consent forms.

    Please go to Buchanan E266, the CIS Main Office, to pick up and sign for your equipment. Please bring any bags or transportation methods that you can as we cannot guarantee that bags will be provided. Returning Equipment: All laptops and equipment must be returned to the CIS Main Office by 4:30pm on the last day of use, or by 8:30am the following business day. Faculty/Staff: if you are unable to return items within this time frame, please contact cis.bookings@ubc.ca to arrange a return time.

    Both forms must be electronically signed and submitted for your lending request to be valid.
  • This policy outlines the expectations for borrowing all equipment held by CIS. This equipment serves the needs of the programs and our community of students, we trust you to treat it with care, as if it were your own, in order to ensure that it will serve others as it serves you.

    Priorities The use of equipment is prioritized according to student need. The general prioritization list is:

    1. Student use for CIS capstone courses (FNIS Practicum, FNEL 389/482)

    2. Student use for other CIS courses

    3. CIS events

    4. Indigenous students with CIS faculty sponsorship

    5. Faculty individual use

    We will not be lending our equipment outside of the department.

    Process

    Staff/Faculty can request for equipment to be signed out for a one week duration by visiting the CIS AIR site, and filling out the equipment request form. The availability of specific items can be checked by referring the appropriate item code to the booking calendar hosted here: www.cisbookings.arts.ubc.ca (also linked on the AIR site).

    If you anticipate needing equipment for your classes, please put in general requests at the beginning of the term. Furthermore, if you foresee students requiring equipment for their course projects, please instruct them to reserve it as early as possible (i.e. when project proposals are due).

    Last minute requests will be accommodated as much as possible, but we are not able to ensure that they ca be fulfilled. Please, whenever possible, give us two-business days’ notice for your equipment needs. If you put a hold on the equipment, please let us know in advance if you will no longer need the equipment so that others can use it.

    Due to the requirements of the FNIS Practicum, the recorders will be reserved for Practicum students for January – February until they submit their proposals in late November. Once they know whether their project requires equipment or not they will inform the office and the relevant items will be held and the rest released for general bookings.

    If you require more time with the equipment, please contact the CIS Main Office before the due date at cis.assistant@ubc.ca to renew your booking. If folks are waiting for equipment, your renewal will not be approved and you will need to return it by your deadline. If you do not bring the equipment back on time, your borrowing privileges may be affected.

    All equipment borrowed for student use for courses must be returned before any coursework is considered complete. Failure to return the items will result in a withheld grade.

  • I accept full responsibility for the equipment while it is signed out to me. The obligation remains in effect until the equipment has been returned in good condition, free from damage or disfigurement. I understand that any damaged, lost, or misplaced equipment will be charged to me. If the equipment is lost or not returned by the return date, I agree to pay for the replacement cost of the loaned equipment. Should any repairs be necessary, I agree to pay for the cost of the repairs. This equipment is the property of UBC CIS and it serves the needs of the program and our community of students, we trust you to treat this equipment with care, as if it were your own, in order to ensure it will serve others as it serves you. If you need more time with the equipment, bring it to the CIS Main Office (E266) on the due date and sign it out again. If you do not bring the equipment back on time, your borrowing privileges may be affected.
